No appointment is necessary. We will accommodate
you as soon as possible, but please remember
that although; everyone is seen at the Urgent CARE
Clinic on a first come, first served basis some patients maybe seen sooner based on the severity of their symptoms. We
do our best to see everyone promptly but due to
the volume and the nature of problems we are
asked to deal with, there may be a wait. It will,
however, be less than the wait of scheduling an
appointment or waiting in the emergency room.

insurance card and any insurance co-pays are due
at the time of service.
Worker's Compensation and patients involved in motor vehicle accidents can only be seen weekdays between the hours of 9:00 am and 5:00 pm.

Medication Assistance Program (MAP)
| |
Silver Health CARE offers their patients a service that may result in free medications to those who qualify. Staffed by volunteers, the Medication Assistance Program (MAP) is open Mondays and Wednesdays from 9:00AM to 12:00 noon and is located in the Silver Health CARE conference room.
In order to apply:
- A referral From available at the doctor's office or at the MAP location must be completed by the patient and his/her doctor.
- Documentation of all household income must be rovided. (ie. copies of Social Security Benefit letter, federal tax return, pension check, or bank statement.
- If the patient has any insurance coverage that covers prescriptions, he or she would likely NOT be eligible for the program. However, often the coverage is in the form of a discount card and the patient may still be eligible.
- Many applications request a listing of monthly medical expenses.
When all the necessary information above is available, a volunteer can assist the patient in determining if their medication is offered in the program and in filling out the specific application for the speecific brand name medication. Each application will be returned to the patient's doctor who needs to sign, attach a prescription and put it in the mail. If everything is in order and the patient qualifies, the medication (usually a 90 day supply) will be sent to the doctor's office or directly to the patient within 3 -6 weeks. Generally, reapplication is necessary every three months. However several companies allow patients to call in for refills.
This service is for the entire community. Other locations are available. If you have any questions, come by Monday and Wednesday, 9:00AM to 12:00 Noon or call 575-538-2981 during those times.
